Please fill out and send with your child on the first day of school:
- Emergency Information Form: Lists approved adults who have permission to pick up your child from school in the event of illness, emergency or your inability to come to school. Listed persons will be required to show a photo ID prior to release of your child
- Responsible & Acceptable Use Policy (RAUP) for District Computer Systems: Your child will not be permitted to use District computers or networks systems without this form.
Other Forms:
- PSG Check Request Form: Please fill out, get the appropriate board member signatures, and attach receipts. Completed forms should be turned in to the treasurer cubby in the Parent Center in the main office.
